Supervision

  • Employers and Supervisors can now utilize the forms above to submit the required verification forms for their supervisees.  To utilize the Submit Online option, the supervisee must have already submitted an application or be applying within 3 months. Forms may not be submitted directly from a supervisee unless they are in a signed, sealed envelope with an application.

The Arizona Statutes/Regulations Tutorial is required to be completed each two year license period pursuant to A.A.C. R4-6-802.

Applying for a License by Examination

Please review the following and then select the Discipline below for which you are applying:

  • Each license type has a checklist in the application to assist you.
  • The Application Form is a fillable .pdf.  For best results, download the form, save and open it with Adobe.  
  • All new applicants must provide verification of an active DPS clearance card or a set of fingerprints on a blue lined FD-258 card with a $40 fee.
  • Applicants using a foreign degree to meet curriculum requirements should review the foreign equivalency rules HERE.
  • Some applicants are eligible for a temporary license.  Review the eligibility requirements HERE.
  • The $250.00 application fee can be paid by cashier's check or money order, or with a debit/credit card by clicking HERE.
  • Applicants for independent licensure will need verification of their supervised work experience and clinical supervision from their employers/supervisors.  The required forms can be found by choosing the Discipline below.

What is the Arizona Board of Behavioral Health Examiners?

To establish and maintain standards of qualifications and performance for licensed behavioral health professionals in the fields of counseling, marriage and family therapy, social work and substance abuse counseling and to regulate the practice of licensed behavioral health professionals for protection of the public.

How do I become a BHP in Arizona?

To be eligible for certification, individuals must complete an annual OSHA compliant Bloodborne Pathogens (BBP) training and have current Child & Adult CPR/First Aid certification. Online BBP training is accepted. However, online-only CPR/First Aid is not valid for this requirement.

What are the requirements to be a BHT in Arizona?

Must have High School Diploma, GED or higher. Must be 21 years old at the time of completion of your certification. 2. Must complete 1000 hours of BHT supervised work.

What is the difference between LAC and LPC in Arizona?

As in many other states, counselors in Arizona are known as Licensed Professional Counselors (LPC). While practicing under supervision, counselors are referred to as Licensed Associate Counselors (LAC).
