I am new to Texas and was applying for a Texas License at the Fort Worth Mega Center. I can’t believe the order, the discipline, the control, and the smooth operating process you have implemented! Everyone there was there to help. No attitudes, no indifferences, no grumpy frowns, just willing employees ready to help! Your Fort Worth location should be an example to America on how local Government facilities should be ran. Your employees are just awesome! The ladies at the entrance made me feel welcome. The ladies doing the eye exams were fabulous. To the person in charge of this facility, congratulations, great job, well done, you hit a home run right out of the park! Thank You!

Birth Certificate, Marriage or Divorce Records: Birth Certificate must: Be issued by the city, county, or state of birth List the applicant's full name, date of birth and place of birth List the parent(s)' full name Have the signature of the city, county, or state registrar Have the date filed with registrar's office (must be within one year of birth) Have the seal of issuing authority Marriage Certificate: Local County Clerk's Office where the license was obtained.
